It is very important that we receive your new client paperwork and pet’s pertinent medical records at least 48 hours prior to your appointment so we better understand your pet’s needs. Please email the forms to calintegrativevetservices@gmail.com. For client privacy reasons we do not request records on your behalf. You must instruct your pet’s primary veterinarian to send us their records. We strongly recommend you review our payment policy section prior to requesting an appointment.
